AB  - Although cytomegalovirus (CMV) is not considered tumorigenic, there is evidence for its oncomodulatory effects and association with hematological neoplasms. Conversely, a number of experimental and clinical studies suggest its putative anti-tumour effect. We investigated the potential connection between chronic CMV infection in patients with B-lymphocyte (B-cell) malignancies in a retrospective single-center study and extracted relevant data on CMV prevalences and the incidences of B-cell cancers the world over.

Comparison of two methods for detecting cytomegalovirus (CMV) shedding by urine in group of patients at risk for active cytomegalovirus infection

AB  - Detecting of viral shedding in urine can contribute (in correlation with other diagnostic parameters) to diagnosis of active cytomegalovirus (CMV) infections in patients at risk. The objective of this study was to evaluate and compare one non-molecular (immunofluorescence) and one molecular assay (in situ hybridization) for CMV detection in urine samples. Urine and blood samples were collected from 18 patients with different clinical diagnosis who were at risk for active CMV infection (transplant recipients, patients undergoing immunosuppresive therapy for other reasons, pregnant women with suspected active CMV infection). Indirect immunofluorescence (IIF) was performed using polyclonal anti - CMV antibodies and anti-IgG FITC-labeled antibodies. In situ hybridization (ISH) was performed using commercial kit (RembrandtÒ - Kreatech diagnostics). Blood samples were tested for presence of anti-CMV IgG and IgM antibodies using ELISA (BioChem ImmunoSystems Italia S.p.A.). Nine out of 18 (50%) patients were found positive by IIF, while ISH found 12 (67%) positive urine samples. Eight samples were positive by both methods, 4 were positive only by ISH and 1 was found positive only by IIF. All patients had IgG anti-CMV antibodies but IgM anti-CMV antibodies were not detected in any blood sample. In situ hybridization had shown disadvantages by being time-consuming, more labor-intensive and more expensive than immunofluorescence but its rapidness compared to culture methods and its higher sensitivity, that our findings supported, makes it the method of choice in cases when fast and proper diagnosis is needed.
AB  - Otkrivanje virusnog izlučivanja urinom može da doprinese (uz ostale dijagnostičke parametre) blagovremenoj dijagnozi aktivne citomegalovirusne (CMV) infekcije kod pacijenata koji su u riziku od ozbiljnih komplikacija citomegalovirusne infekcije. Cilj ovog rada bio je da oceni vrednost i uporedi jednu nemolekularnu (imunofluorescencija) i jednu molekularnu (in situ hibridizacija) tehniku za otkrivanje CMV u urinu. Uzorci urina i krvi sakupljeni su od 18 pacijenata sa različitim kliničkim dijagnozama koji su bili u riziku od aktivne CMV infekcije (primaoci transplantata, pacijenti na imunosupresivnoj terapiji iz drugih razloga, trudne žene kod kojih postoji sumnja na aktivnu CMV infekciju). U tehnici indirektne imunofluorescence (IIF) korišćena su poliklonska anti-CMV antitela i anti-IgG antitela obeležena FITC-om. Za in situ hibridizaciju (ISH) korišćen je komercijalni kit (RembrandtÒ- Kreatech diagnostics). IgG i IgM ELISA (BioChem ImmunoSystems S.p.A.) su korišćene za otkrivanje prisustva anti-CMV antitela u serumu istih pacijenata. Devet od 18 (50%) pacijenata je nađeno pozitivno tehnikom IIF, dok je tehnikom ISH otkriveno 12 (67%) pozitivnih. Osam uzoraka je bilo pozitivno korišćenjem obe tehnike, 4 je bilo pozitivno samo tehnikom ISH, dok je 1 nađen pozitivan korišćenjem samo tehnike IIF. Svi pacijenti su imali IgG anti-CMV antitela, dok ni jedan pacijent nije bio pozitivan na prisustvo anti-CMV IgM antitela. Pokazalo se da je tehnika in situ hibridizacije dugotrajnija, skuplja i teža za izvođenje od tehnike indirektne imunofluorescencije, ali njena brzina u poređenju sa izolacijom virusa u kulturi ćelija i njena velika osetljivost, koju i naši nalazi podržavaju, čine je metodom izbora u slučajevima kada je neophodna brza i verodostojna dijagnoza.
